Resource Specialist – Men’s Therapeutic Community – Pueblo Salary: $28,000 per year SUMMARY: Under general supervision, employee maintains an assigned caseload of clients ensuring effective delivery of care following facility policies/procedures and required Department of Corrections “DOC”, Pueblo County, Division of Criminal Justice (DCJ) and Department of Behavioral Health (DBH) standards and contractual obligations. ESSENTIAL DUTIES AND RESPONSIBILITIES: include, but are not limited to, the following. Other duties may be assigned. Creates, updates and maintains case records on each assigned client. Conducts assessments using comprehensive assessment tests such as Standardized Offender Assessment – Revised “SOA-R”, intake/assessment forms, referral documents and observable behavior May conduct educational lectures or seminars. Facilitates all scheduled Peer Intense Process Group (otherwise known as Games); Collects client history such as psychiatric, medical, history of escape, violence, suicide, etc. and develops recommended individualized treatment plan; Identifies clinical issues such as history of violence, escape, medical, etc. and prioritizes which issues are appropriately addressed during treatment process; Works with a team to develop/recommend a comprehensive treatment plan, generating problem statements, goals and objectives, measurable/quantified treatment program success; Writes and updates case file documentation of the individualized Case Management Supervision Plan (treatment plan), intervention efforts taken and continuous quality improvement initiatives; Conducts ongoing assessments, interventions, confrontation, individual and group lectures and reports client treatment progress; Participates in multidisciplinary case conferences and treatment plan reviews; Completes monthly Structured Progress Feedback reports and other monthly reports as required to track client progress as well as discharge/termination reports/summaries; Develops continuing care treatment plans/strategies for clients prior to discharge as well as aftercare plans. Coordinates and refers to supportive services in the community to supplement treatment and counseling. Referrals to vocational, financial, education, housing, mental health, medical and domestic violence services when needed. Assists clients in obtaining services to become self-sufficient and avoid relapse. May be required to observe clients during the collection of urine samples and assures compliance with chain of custody process. Includes proper charting and reporting. QUALIFICATIONS: Education: Experience: Other: Bachelor’s degree in human services field. Master’s degree preferred. One or more years working in human services and/or criminal justice field. Experience working in therapeutic community programs preferred. Must have an understanding of SOA-R level of care criteria. Must have solid communication skills. Good computer skills plus word processing and spreadsheet software skills such as MS Word, MS Excel and MS Office, as well as Internet navigation skills. CPR and first-aid certification. (Initial training and renewals paid for by agency). Valid driver's license and insurability. PHYSICAL DEMANDS: Position requires intermittent sitting, standing and walking with occasional lifting of up to 20 lbs. and good visual and auditory acuity for monitoring client behavior.
