Analytic Geometry EOC Intervention Project
This project is designed to help students prepare for the Analytic Geometry End-of-Course Test. The
project is a strategic individualized plan that will help students be successful. This completed form is
due by EOC test day: Monday, May 4, 2015
Instructions for completing the project:
1. Students must complete a diagnostic test by the end of class on Thursday 3/12/15. Take the
test one time and record original results in the table below.
2. Students will complete USA Test Prep, Study Island, and Khan Academy online assignments.
3. Students will take a post-test on end of class on April 23rd. Record results in the table below.
Post-test results will drive the individual focus for the final week of preparation.
Important notes:
 Students have been given the opportunity and time in class to establish and test all accounts
required for successful completion of this project.
 Time spent on lessons will be factored into student grades. Results MUST be submitted
electronically and recorded on this document.
 You must be logged into Khan Academy before clicking on links. Khan Academy assignment
files are located on the geometry tab of the teacher’s website.
