DETENTION ALTERNATIVE PROGRAM
ANKLET COMPONENT
You have been placed on the Detention Alternative Program (DAP), as an alternative to being detained at the
Allen County Juvenile Center (ACJC) or Youth Services Center (YSC).
The following rules shall be followed or your conditional release will be revoked. If that were to occur, you
will return to Court for a review of your detention status which could result in you being detained at either the
ACJC or YSC.
1. You must be in your home at all times unless you have permission to leave from the DAP office.
2. You will be granted permission to leave your home for the following reasons:
a. attend school,
b. attend your current approved job,
c. attend one church service per week,
d. attend probation and/or counseling appointments,
e. attend evening educational classes,
f.
attend medical appointments.
3. You are not to leave Allen County while on the DAP Program.
4. You must attend school regularly. If you are ill, absent or tardy, you MUST call 449-8140 no later than
8:45 a.m. on the day of your absence.
5. You must follow all school rules and regulations.
6. It is a violation of the DAP Program if you earn ANY type of disciplinary action, (out-of-school or inschool suspension, detention, Saturday School, etc.) and you must report any disciplinary action to the
DAP office immediately.
7. You must keep all appointments with your probation officer and attend all court hearings as notified.
8. You must not be involved in unlawful behavior.
9. You shall not consume or possess alcohol, inhalants, controlled or mind-altering substances or drugs, or
be present at any place or in any vehicle where alcohol, inhalants, controlled or mind-altering substances
or drugs are present.
10. You shall not possess any firearm, loaded or unloaded, or any knife, weapon, device, equipment,
chemical substance or other material that could be used to cause serious bodily injury to another person.
11. You must follow all lawful and reasonable requests of your parent/guardian. The parent/guardian may
terminate the program at any time pending judicial review.

12. DO NOT call ACJC to request permission to leave your home. None will be granted.
13. You and your family must cooperate with DAP officers and allow them into your home at any time of
the day or night to ensure DAP compliance.
14. This program is effective immediately.
15. You may be subject to random drug screens and alcohol testing.
16. You must contact the DAP office by 4:00 p.m. with any request for permission to be out of your home.
17. You must contact the DAP office by 4:00 p.m. Wednesday with any request to be out of your home on
Friday, Saturday or Sunday.
18. You will not be granted permission to seek employment. However, you MAY be granted permission to
attend an interview to secure employment.
19. You are not allowed to have any answering machines, cordless phones or computers connected to the
same phone line as the monitoring equipment.
20. You may not have any special features on your phone. (For example, call waiting, call forwarding, call
block, caller I.D, etc.)
21. The monitoring equipment MUST NOT be disconnected or moved.
22. You and your family must limit all phone calls to 10 minutes, with a minimum of 15 minutes between
calls.
23. You must immediately contact the DAP Office if you are having problems with your phone line or the
monitoring equipment.
THE DAP TELEPHONE NUMBER IS 449-8140 and is available Monday - Friday, 8:00 a.m. to 12:00
p.m. and 1:00 p.m. to 4:30 p.m., excluding Holiday’s.
In the event of an emergency, contact ACJC at 449-8449. This is a 24-hour, 7-day-a-week number. Ask
to speak to a DAP officer.

INFORMATION TO THE JUVENILE/PARENT:
The Anklet will not be harmed by normal human actions and may be exposed to water (shower only), heat,
cold, etc., without concern. You may not remove the anklet unless you are told to so by a Probation Officer. It
is a violation of this Program to remove or attempt to remove the Anklet.
HOW DOES THE MONITORING DEVICE AND ANKLET WORK:
1.
You must let the phone ring two (2) times before answering a call.
2. Call the DAP Office or the Allen County Juvenile Center (ACJC) immediately if there are any problems
with the phone line or equipment.
3. Do not attempt to remove the Anklet or tamper with the band unless authorized to do so by a Probation
Officer. Removal should only be performed by a Probation Officer. Exceptions for medical or other
reasons should be reported immediately to the DAP Office or ACJC. DO NOT MOVE THE
EQUIPMENT. It is very sensitive and moving it may result in a violation. If the equipment needs to
be moved for any reason, notify the DAP Office or a DAP Officer.
4. No phone calls can last more than ten (10) minutes and there must be a period of at least fifteen (15)
minutes between calls.
5. There are to be no computers connected to the same phone line as the monitoring equipment.
NOTE: ALL ANKLETS and MONITORING EQUIPMENT are the property of the Allen Superior Court.
Your acceptance of the use of the ANKLET and MONITORING EQUIPMENT as a condition of your
release confirms you’re agreeing to neither open nor abuse this equipment. Opening, destroying, or loss
from theft of either the Anklet or the monitoring equipment constitutes a crime punishable under the law
and is a violation of the Court Order.
I acknowledge receipt of the below listed FMD monitoring unit and anklet. I further understand that the
FMD monitoring unit will be installed in my place of residence and the anklet will be worn on my person
while I am being supervised under the Detention Alternative Program.
I also understand that failure to return the equipment in good working condition at the completion or
termination of this Program to the Allen County Juvenile Probation Department will constitute a violation. I
further understand that I could be charged for the repair or replacement of any part of the monitoring
equipment or anklet and criminal prosecution may result.
By signing below, I acknowledge that all equipment to be in good working order. Any notations regarding
the condition of the assigned equipment will be completed at the bottom of the page by a DAP Officer.

understand that any violation of the DAP Program may result in me being detained at either ACJC or YSC
pending further order of the Court.

obeying these rules, and will keep the DAP officers informed of all violations. I understand that failure to
follow the above rules will result in my child’s conditional release being revoked and he/she may be
detained at either the ACJC or YSC pending further order of the Court.
In addition, as parent(s)/guardian(s) of said child, I/We understand that I/We are responsible for loss and/or
damage to either the Anklet or the monitoring equipment.
