Playground Installers ADA Compliance for OPS Playground Installation
This form is to be completed by the agent and installer for the selected playground equipment company and the school
principal. Signature approvals must be obtained for sections of this form from OPS Schoolhouse Planning,
Environmental/Safety Department and the Maintenance Department before installation can proceed.
Before completing this form, read and review the OPS Procedures for Purchase of Playground Equipment.

1. Required Item - Certified and Approved Playground Installer
Identify the approved certified playground installers from the OPS hyperlink located under the Environmental/Safety
webpage tab "Playgrounds". Contact two or more installers for bids on the equipment you want. Then choose an
Installer to purchase your playground equipment from.
I certify that the Playground Installer and the company they represent are approved by Omaha Public Schools
Environmental/Safetv Department and comply with the insurance requirements of the District.

2. Required Contact - Schoolhouse Planning for Approval of the Playground Installation Location
Schoolhouse Planning will be responsible for identifying and approving a suitable location where playground
equipment will be installed. The School Principal will need to contact the Director of Schoolhouse Planning (SHP) at
402-557-2800 or 531-299-0180 and request that Schoolhouse Planning check the proposed playground equipment location
to make certain that it is not over any underground piping, heat wells, electrical or other apparatus in such a way that
would create damage to existing items or block any maintenance access points. After the SHP Director or their designee has
reviewed the location/design plans, this portion of the form must be signed by someone in the SHP Department.
I certify that the location chosen for this playground equipment and its design has been reviewed by the
Schoolhouse Planning Department. We approve that this equipment can be safely installed at the selected location.

3. Playground Installer Requirements- Insure the Design and Layout Complies with ADA Regulations and
OPS Standards
The playground installer will know, agree to, and follow all applicable ADA regulations and OPS Standards for the
design, purchase, and install of playground equipment. The Installer will also serve to advise the Principal at the
location receiving the playground equipment and other involved OPS personnel as need arises. Refer to OPS Procedures
'
for Purchase of Playground Equipment for more detailed Information.
I certify that the design, layout and equipment chosen by the school Principal has been reviewed and is in
compliance with ADA Accessibility Guidelines as set out by the Federal Justice Department.

4. Required item - Environmental/Safety Management - ADA Review of Location and Equipment Plans
Contact the Environmental/Safety Department Manager to set up arrangements for a review the intended location and
design of the playground equipment. The Department will also assess whether the design has the correct equipment
elements to provide required ADA access for play activities.
I certify that the design and equipment has been reviewed by the Environmental/Safety Department and is
acceptable to meet ADA access requirements.

5. Please check one of the installation methods listed below for the set up of playground equipment.
Contract with a playground installer, approved by OPS, to do all work necessary to install equipment including
concrete access sidewalk, transfer points, containment borders and ground surface material. All work necessary must be
contracted with installer prior to purchase of playground equipment.
Installation of playground equipment by volunteers with supervision provided by a certified installer, approved by OPS.
All work done by volunteers including concrete access sidewalk, transfer points, containment borders and ground surface
material.
Double check to make certain you have checked the box to indicate one of the installation methods listed above.
OPS Maintenance Division may agree to install concrete access sidewalk, concrete base for transfer points, containment
border and ground surface material. The installation method you checked above will be the one used to install the
new playground equipment on our school playground.
