J.B. Mouton, LLC Subcontractor Safety Policy Statement of Minimum Expectations for Subcontractors A. STATEMENT OF POLICY It is the policy of this company to provide a healthy and safe place of employment for all subcontractors, to abide by all regulations, as they pertain to our industry, which are set forth in federal, state and local standards and statutes and to integrate J.B. Mouton, LLC’s General Safety Rules (described below) into every aspect of our business activity. Subcontractors must also abide by all policies and procedures in J.B. Mouton, LLC’s Safety Manual which is available upon request. GENERAL SAFETY RULES ACCIDENTS OR INJURIES, no matter how minor, shall be reported to J.B. Mouton, LLC’s Superintendent, Foreman or Project Manager for immediate attention. An accident is any unforeseen or unexpected event that may or may not result in injury or damage to property or equipment. FALL PROTECTION, Personal fall arrest systems shall be used at all times when workers are working on aerial or scissor lifts and when working 6 feet above the ground if other protection systems are not in place. FORKLIFT & CRANE CERTIFICATION, Any workers operating forklifts or cranes shall be trained and certified on the equipment. Workers must be able to show evidence of training or they will not be allowed to operate the equipment. GOOD HOUSEKEEPING, shall be practiced on all construction projects. Work areas, aisles and walkways shall be kept clear of loose material and tools. Protruding nails, wires and bolts shall be bent over or cut immediately. Debris shall not accumulate and shall be removed frequently. Always keep your area clean. HARD HATS, shall be worn on all jobs by all personnel at all times. No exceptions. EYE & FACE PROTECTIVE EQUIPMENT, which is required and made available shall be worn when the nature of the operation presents a potential eye or face injury. These operations include but are not limited to grinding, chipping, welding, sawing or drilling in concrete and handling of acids or corrosive materials. SHOES shall be first grade, hard sole, with a heel and be ankle high. Steel toe shall be worn as needed when dangers to the toes are present. CLOTHING shall be appropriate for the hazard associated with the duties performed and shall not include cuffed trousers, short pants, torn or loose clothing and shall be in good repair and include a body-covering shirt. GLOVES shall be worn when handling rough edged or abrasive materials or where hands are subject to lacerations, punctures, burns or other injuries. FLOOR AND ROOF OPENINGS shall be planked over or barricaded by guardrails and toe boards. Employees shall maintain these protective planks and barriers and shall not disturb or remove them except as directed by the supervisor. If temporary removal is required, constant guard of openings is required until barrier is replaced. Employees removing these barriers shall be responsible for their replacement. All planking for floor openings shall be nailed or cleated in place. EDGES OF BUILDINGS shall be provided with guardrails and toe boards on all elevations according to OSHA standards. These protective barriers shall not be removed except as directed by the supervisor and in accordance with OSHA Standards. SCAFFOLDS AND PLATFORMS shall be constructed of lumber or metal in accordance with safety standards. Due to the varied kinds and uses of scaffolds and platforms, the job supervisor should be consulted prior to erecting them. Guardrails and toe boards are required on scaffolds and platforms according to standards. LADDERS shall be according to approved standards and used as directed. Revision # 1 – 3/20/14 Page 1 of 3 HAND TOOLS shall be kept in safe condition and shall not be used for anything other than their intended use. POWER TOOLS shall be operated only by authorized personnel, with guards furnished by the manufacturer "in place," and if electrical, shall be grounded or double insulated. All temporary power connections shall utilize GFI protected receptacles. POWDER ACTIVATED TOOLS shall be used only by delegated persons who have been instructed and trained in their safe use. COMPRESSED GAS CYLINDERS shall be chained or properly secured in an upright position and shall be placed in cylinder carts whenever being transported, whether to different locations on the project or to or from a project. SOURCES OF IGNITION shall be prohibited in areas where flammable liquids or explosives are stored or issued, and appropriate warning signs shall be posted at these locations. Approved safety cans shall be used when transporting flammable liquids on the job site. PROTECTION FROM FALLING OBJECTS shall be provided by storing materials at least 6 feet from hoistways or inside floor openings and at least 10 feet from an exterior wall which does not extend above the material. Excavated or other material shall not be stored near the edge of an excavation. "Men Working Above" signs shall be posted at the lower levels by the employees working above other workmen who may thus cause them to be exposed to falling objects. INTOXICATING BEVERAGES, possession or use of, before or during working hours is strictly forbidden. RIDING OF EQUIPMENT PROHIBITED No person shall ride any hook, hoist or other material handling equipment. MACHINERY, WHEN OPERATING, shall not be oiled, cleaned, adjusted or refueled. WELDING AND BURNING OPERATIONS shall be conducted only by authorized personnel under proper supervision. Approved personal protective equipment including fire extinguishers shall be provided and used. Localized ventilation shall be used in confined spaces. EXCAVATION AND TRENCHING shall, when there is a danger of slides or cave-ins and where more than 5 feet in depth, be shored, braced or sloped to an angle of repose consistent with the material being excavated. MATERIAL, EQUIPMENT, LUMBER, etc. also shall be secured where it is at risk to be blown from a roof or high place during a windstorm. HORSEPLAY OR PRACTICAL JOKES by any individual could be hazardous to others and will not be permitted on job site. Violators will be subject to disciplinary action. FOR YOUR PROTECTION report all unsafe conditions and unsafe acts to your supervisor. Make sure all equipment, tools or working platforms, scaffolds, ladders and walkways are safe prior to use. PROTECTION OF MEMBERS OF THE GENERAL PUBLIC: Protection of the lives and property of members of the general public shall be considered paramount to all factors and functions of our operations. We will make every attempt possible and practical to keep from injuring the public by using barricades, warning signs, lights, nets, etc., where our operations might expose them to injury. All visitors not associated with the project by virtue of some responsibility for any part of it (e.g., owner, architect, engineer, subcontractor, supplier, etc.) are prohibited from being within the project construction limits and shall be directed to leave the jobsite premises. Never hesitate to ask a bystander, a neighborhood child, a salesman or similar person to leave the jobsite. All company personnel are expected to report visitors to the supervisor and to warn invited guests (e.g., owners, architects) of the hazards and provide them with necessary safety equipment. To aid in warning the general public, a sign shall be required at all entrances to project sites. The wording below is one example of suggested language for such signs. The first warning will be oral and the second warning will be written. rd th th 2. 3 , 4 & 5 Violation – Your Company will be fined $100 per violation and the violation will be written. th 3. 6 Violation – Your worker will be asked to go home for 3 days. th 4. 7 Violation – Your worker will not be allowed back on the job site. *Immediate removal of the employee is always an option depending on the severity of the violation. This will be decided by J.B. Mouton, LLC management. *The above mentioned violations are for each subcontractor’s employee in lieu of the subcontracting company as a whole. C. SUBCONTRACTOR RESPONSIBILITIES All subcontractors of J.B. Mouton, LLC and their personnel are, by contract, subject to this safety policy while working on J.B. Mouton, LLC projects and shall be responsible to the limit of State and Federal statutes for their acts of omission and commission regarding safety. Mouton, LLC Subcontractors Safety Policy The J.B. Mouton, LLC Subcontractor Safety Policy describes important information about J.B. Mouton, LLC and I understand that I should consult my Superintendent or the Safety Coordinator regarding any questions not answered in the policy. I have received a copy of the J.B. Mouton, LLC Subcontractor Safety Policy and understand that it contains important information about J.B. Mouton, LLC Safety policies and about my compliance and obligations as a subcontractor. I acknowledge that I am expected to read, understand, and adhere to J.B. Mouton, LLC Subcontractor Safety Policy and to familiarize myself with the material in the J.B. Mouton, LLC Subcontractor Safety Policy, and that J.B. Mouton, LLC may change, rescind, delete, or add to any policies and practices described in the J.B. Mouton, LLC Subcontractor Safety Policy from time to time, at its sole and absolute discretion, with or without prior notice. J.B. Mouton, LLC will advise subcontractors of material changes within a reasonable time. I understand that revised information may supersede, modify, or eliminate existing policies and practices. Only the President of J.B. Mouton, LLC has the authority to adopt any revisions to the policies in this handbook. I received a copy of the J.B.
