As a client, you have the right to: Be Heard Be Informed • Be listened to and ask questions. • Be an active participant in making decisions about the services you receive. • Provide feedback, positive or negative about your treatment experience. • Receive information to help you make decisions in the planning and delivery of your treatment services. As a client, you have the responsibility to: Be Respectful Be respectful of other clients, volunteers, staff members and Centre property. Act and communicate in a considerate manner – including avoiding harassment and bullying as well as discrimination based on age, gender, ability, class, ethnicity and sexual orientation. Actively Participate in Your Care Provide accurate and complete information to the Centre so that staff can provide the best possible care and services for you and your family. Adhere to established residential and aftercare program rules and structure. Follow the 6-month treatment plan developed in consultation with you, to the best of your ability.
