Scholarship Application
Camp Dates: June 18-22, 2016
Who is Eligible?
Rising 6th, 7th and 8th grade male students are eligible for Roy Williams Carolina Basketball Camp
Scholarship. Each winner will receive an all-expense paid scholarship to attend the Roy Williams
Carolina Basketball Camp this summer.
Instructions:
1. Write a brief (approximately 300 words) essay from one of the two topics provided.
2. Answer the extra credit question.
3. Complete the scholarship application form.
4. Have parent/guardian sign the Parental/Guardian Consent portion of this application. All
applicants must have a parental/guardian signature in order to be eligible for the scholarship.
Contest Guidelines:
 The contest is open to rising 6th, 7th and 8th grade male students in middle school. This scholarship
is not available to students entering the ninth grade.
 Criteria for contest include:
- Originality and accuracy of written essay: 55%
- Scholastic and extracurricular activities: 40%
- Extra credit question: 5%
 Forms must be completed and returned by March 31, 2016.
 Complete forms using ink, typewriter or attach computer printout. Do not use pencil!

chosen as a Jones-Onslow EMC Touchstone Energy Camper.
___I do hereby commit to provide transportation for my child to and from the Roy Williams Carolina
Basketball Camp, located at UNC Chapel Hill, North Carolina on June 18-22, 2016, should he be selected
to attend.
___I have checked our family’s calendar and know that, should my child win this scholarship, he will be
available to attend the camp on June 18-22, 2016.
___I understand that if my child cannot meet these commitments he will not be eligible for the
scholarship.
