RIDING PARK WAIVER FORM FOR MINOR CHILDREN
Note: This Form Must Be Executed By the "Natural Guardian" of any
Person Under the Age of Eighteen (18) to Participate in any Motor
Sport Activity at this Location.
NOTICE TO THE MINOR CHILD'S
NATURAL GUARDIAN
READ THIS FORM COMPLETELY AND CAREFULLY.
YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY
DANGEROUS ACTIVITY.
YOU ARE AGREEING THAT, EVEN IF LAZY SPRINGS, LLC,* USES REASONABLE
CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE
SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE
THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE
AVOIDED OR ELIMINATED.
BY SIGNING THIS FORM, YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR
RIGHT TO RECOVER FROM LAZY SPRINGS, LLC,* IN A LAWSUIT FOR ANY
PERSONAL INJURY, INCLUDING DEATH TO YOUR CHILD OR ANY PROPERTY
DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE
ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND LAZY
SPRINGS, LLC,* HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF
YOU DO NOT SIGN THIS FORM.
* Note: Lazy Springs, LLC, and the protection from liability afforded to Lazy Springs, LLC.
pursuant to this Waiver Form, shall include, but is not limited to, Lazy Springs, LLC, and all
related parties, company officers, directors, elected officials, agents, employees, event
volunteers, racing associations, participants, inspectors, surveyors, consultants, sponsors,
facility operators, owners of equipment used at the facility, and owners of land used where
the facility is located.

I certify that I am the Natural Guardian (as defined in Section 744.301 Florida Statutes) of the
minor child subject to this Agreement and that I am authorized to execute this release on
behalf of said minor child. I understand that I am affirming that I am the Natural Guardian of
the minor child and such affirmation of being the Natural Guardian is being relied upon by
Lazy Springs, LLC, in allowing the minor child to participate in the activity.

744.301 Florida Statutes defines "Natural Guardians" as: "(1) The mother and father jointly are natural
guardians of their own children and of their adopted children, during minority. If one parent dies, the
surviving parent remains the sole natural guardian even if he or she remarries. If the marriage between the
parents is dissolved, the natural guardianship belongs to the parent to whom custody of the child is awarded. If
the parents are given joint custody, then both continue as natural guardians. If the marriage is dissolved and
neither the father nor the mother is given custody of the child, neither shall act as natural guardian of the child.
The mother of a child born out of wedlock is the natural guardian of the child and is entitled to primary
residential care and custody of the child unless a court of competent jurisdiction enters an order stating
otherwise."
