Investigator Viability/Disclosure: I, ____________________________ the undersigned and hereby agree to
employ Mark Neyer owner of Neyer Detective Agency a Licensed & Insured as a Private Investigator in San Jose
California to conduct my investigation which is confidential & agrees to hold the same confidence except for legal
permissible reasons in compliance with State, Local and Federal laws and regulations. The information provided
will not be used to cause emotional or physical harm to the subject & I agree not to use the information for any
criminal act, harassing or stalking. As the client we assume any & all personal responsibility for any violation of
the provisions or any damage arising from my actions. The goal of this investigation is to ascertain the true facts
of the case & the specific details of this investigation i.e.: Litigation, marital, civil action, divorce, criminal action,
background investigation, pre-marital background investigation, pre-marital investigation, trial pre-preparation,
missing person, etc. I agree Neyer Detective Agency and Owner Mark Neyer will not assume any responsibility
for any facts discovered during the investigation, now or in the future & will pay for any reasonable attorney's fees.
I understand Private Detective work & investigative work is not magic, it is understood that the investigator shall
use his best efforts to investigate this matter set forth. I understand the results cannot be guaranteed as defined
in the terms and conditions.
Neyer Detective Agency retains the right to provide or not to provide you with personal information obtained
during the course of the investigation & will be handled in a one on one basis using our discretion. I agree to
indemnify & hold harmless Neyer Detective Agency and/or owner Mark Neyer for errors on data bases or
documents, in addition we agree not to indemnify, hold harmless its agents/employees for any & all damages,
actions, causes of actions, demands, acts of terrorism either domestic or internationally, client's misconduct,
misinformation/miscalculations, acts of God, subject's elements, acts or factors which are not in control of Neyer
Detective Agency for now and will not be held liable in the future . Any information we provide you will not be
used for any, all illegal purposes & is punishable Local, State and Federal Law's.

Investigative Costs: I the below undersigned agree to pay Neyer Detective Agency a retainer in the amount of
$____________. Our standard investigative rate is $_____________ per hour with a 4 hour minimum per day. In
addition I agree to a mileage expense of $0.55 per mile & any other expenses such as: Travel, report time,
copies or records, court costs, videotape or photograph dubbing and all expenses unless otherwise discussed
and agreed upon. No additional fees or costs will be charged without written and/or verbal authorization. This
includes the completed investigation and/or recommendations. As these types of investigations are very sensitive
with the current Local, State & Federal laws we reserve the right to withhold some information. Initial: ______

Balance Due: I the below undersigned agree to pay Neyer Detective Agency for any remaining balance due
within 10 working days, furthermore I agree to pay 5% compounded every 30 calendar days unless discussed
otherwise. We agree Neyer Detective Agency will have the right to retain & not provide privacy information on the
subject's companions and agree as the undersigned.
