Cost: Free
LUNCH- We will be providing bag lunches for the students to eat in the picnic area.
If you choose to pack a lunch, please make sure it is completely disposable (NO
LUNCH BOXES OR BOWLS)!
Chaperones: $6.00 each (you will need to provide your own lunch) (If you have an annual
pass, you may enter the zoo at the main entrance with your pass.)
***PLEASE NOTE: All Chaperones/parents attending must have a 2016-2017 Background Check on file.
Please see your child’s teacher.***
Please sign and return the bottom half of this permission slip by Monday,
April 17, 2017, in order for your child to attend.

the field trip to the Cincinnati Zoo on Wednesday, April, 19, 2017 with his/her
kindergarten class. I understand that field trips are a privilege and my child’s
behavior directly impacts how others view Newport Primary School! We
must set a positive image in our community! With that being said, by signing
below, you are agreeing that if your child does not have good behavior or
attendance leading up to the field trip day, he/she will not be allowed to
attend.
