1. I accept for use “as is” the equipment listed on this form, and accept full financial responsibility for the care of
the equipment while it is in my possession. I will be responsible for the replacement at full value as determined

4. I understand that use of a helmet designed for recreational snow sports will help reduce the risk of some types
of injuries to the user at slower speeds. I recognize that serious injury or death can result from both low and
high-energy impacts, even when a helmet is worn.
5. I understand and am aware that using the equipment I have rented is inherently dangerous and involves
inherent risks of injury to any and all parts of my body. I hereby freely and expressly assume and accept any and
all risks of injury or death to the user of this equipment while participating in a snow sport.
6. I AGREE TO RELEASE The Rock Sports Complex, LLC (“The Rock”) and MKE Sports & Entertainment, LLC, its
employees, owners, affiliates, agents, officers, directors, and the manufacturers and distributors of the
equipment (collectively “Providers”) from all liability of injury, death, property loss and damage which results
from the fitting, provision or use of the equipment rented under this form, even if such conduct by “the
Providers” is negligent.

2. I have made no misrepresentations to The Rock with regard to any information, which they have requested of
me related to the rental and/or use of the equipment.
3. For Skiers, I understand that bindings may not release at all times or under all circumstances where release
may prevent injury or death. It is not possible to predict every situation in which bindings will release. For
Snowboarders, I understand that the snowboard has a NON-RELEASE BOOT RETENTION SYSTEM and, as a result,
the boot retention system and operation of the equipment, there are inherent risks involved. I understand the
equipment will NOT RELEASE during use, nor is it specifically designed to release as result of forces induced
during ordinary operation, and is therefore absolutely no guarantee of my safety. This equipment-boot-binding
system does not REDUCE THE RISK of injuries to any part of my body.
