PURPOSE/SCOPE
Thepurposeofthisprogramistoprovidegoodpractices,procedures,andtrainingto
employeeswhoworkonelevatedsurfacesandladders.ThisprogramappliestoallUniversity
employeesthatperformdutiesonelevatedworksurfaceswithapotentialfallhazardof6feet
ormore.
2.0
DEFINITIONS
AERIALLIFTDEVICE:Equipmentsuchaspoweredplatforms,vehicle-mountedelevatedand
rotatingworkplatforms,extensibleboomplatforms,aerialladders,articulatingboom
platforms,verticaltowersandpoweredindustrialtruckplatforms.
ANCHORPOINT:Asecurepointofattachmentforlifelines,lanyardsordeceleration
(grabbing)devices.
BODYHARNESS(alsoreferredasFull-bodyharness):Aninterconnectedsetofstrapsthatmay
besecuredaboutapersoninamannerthatdistributesthefallarrestforcesoveratleastthe
thighs,pelvis,waist,chest,andshoulderswithameansforattachingtheharnesstoother
componentsofapersonalfallarrestsystem.
COMPETENTPERSON:Onewhoiscapableofidentifyingexistingandpredictablehazardsin
thesurroundingsorworkingconditionswhicharehazardous,ordangeroustoemployees,and
whohastheauthoritytotakepromptcorrectivemeasurestoeliminatethem.
DECELERATIONDEVICE:Anymechanism,suchasarope,grabbingdevice,ripstitchlanyard,
speciallywovenlanyardorautomaticself-retractinglifeline/lanyard,whichservestodissipate
asubstantialamountofenergyduringafallarrest,orotherwiselimitstheenergyimposedon
anemployeeduringfallarrest.
DECELERATIONDISTANCE:Theadditionalverticaldistanceafallingpersontravels,excluding
lifelineelongationandfreefalldistance,beforestopping,fromthepointatwhicha
decelerationdevicebeginstooperate.
DESIGNATEDAREA:Aspacewhichhasaperimeterbarriererectedtowarnemployeeswhen
theyapproachanunprotectedsideoredge,andservesalsotodesignateanareawherework
maybeperformedwithoutadditionalfallprotection.
FIXEDLADDER:Anyladder,includinganindividualrungladder,whichispermanently
attachedtoastructure,building,orequipment.
GUARDRAIL:Averticalbarriererectedalongtheopenedgesofaflooropening,wallopening,
ramp,platform,runway,orotherelevatedareatopreventfallsofpersons.
LADDER:Adevicetypicallyusedtogainaccesstoadifferentelevationconsistingoftwoor
morestructuralmemberscrossedbyrungs,steps,orcleats.
LANYARD:Aflexiblelineofropeorstrapthatgenerallyhasaconnectorateachendfor
connectingthebodyharnesstoadecelerationdevice,lifelineoranchorpoint.
LIFELINE:Acomponentconsistingofaflexiblelineforconnectiontoananchorageatoneend
tohangvertically(verticallifeline),orforconnectiontoanchoragesatbothendstostretch
horizontally(horizontallifeline).Thisservesasameansforconnectingothercomponentsofa
personalfallarrestsystemtotheanchorage.
LOWSLOPEROOF:Aroofhavingaslopeoflessthanorequalto4in12(verticalto
horizontal).Aroofwithapproximatelya19.5-degreeslopeorless.
LOWERLEVELS:Thoseareasorsurfacestowhichanemployeecanfall.Suchareasinclude,
butarenotlimitedto,groundlevels,floors,platforms,ramps,runways,excavations,pits,tanks,
material,water,equipment,structures,orportionsthereof.
OPENING:Agaporvoid30inchesormorehighand18inchesormorewideinawallor
partition,throughwhichpersonnelcanfalltoalowerlevel.

POSITIONINGDEVICESYSTEM:Abodyharnesssystemriggedtoallowanemployeetobe
supportedonanelevatedverticalsurfacesuchasawallandworkwithbothhandsfreewhile
leaning.
PERSONALFALLARRESTSYSTEM:Asystemusedtoarrestanemployeeinafallfroma
workinglevel.Itconsistsofananchorage,connectors,andbodyharnessandmayincludea
lanyard,decelerationdevice,lifeline,orsuitablecombinationsofthese.
ROPEGRAB(grabbingdevice):Adecelerationdevicethattravelsonalifelineand
automatically,byfriction,engagesthelifelineandlockstoarrestafall.
SCAFFOLD:Anytemporaryelevatedorsuspendedplatform,anditssupportingstructures,
usedforsupportingemployeesormaterialsorboth.
SELF-RETRACTINGLIFELINE/LANYARD:Adecelerationdevicecontainingadrum-wound
linewhichcanbeslowlyextractedfrom,orretractedonto,thedrumunderminimaltension
duringnormalmovementandwhich,afteronsetofafall,automaticallylocksthedrumand
arreststhefall(usuallywithintwofeetorless).
STANDARDRAILING:Averticalbarriererectedalongexposededgesofaflooropening,wall
opening,ramp,platform,orrunwaytopreventfallsofpersons.
SNAPHOOK:Aconnectorconsistingofahook-shapedmemberwithanormallyclosedkeeper,
orsimilararrangement,whichmaybeopenedtopermitthehooktoreceiveanobjectand,
whenreleasedautomaticallyclosestoretaintheobject.
TOEBOARD:Averticalprotectivebarrieraffixedtothescaffoldorplatformfloorthatprevents
tools,materialandequipmentfromfallingtolowerlevels.
TIE-OFF:Aprocedureofconnectingdirectlyorindirectlytoananchoragepoint.
UNPROTECTEDSIDESANDEDGES:Anysideoredge(exceptatentrancestopointsofaccess)
ofawalking/workingsurface,e.g.,floor,roof,ramp,orrunwaywherethereisnowallor
guardrailsystematleast42incheshigh.
VERTICALLIFELINE:Averticallyhangingflexiblelineforconnectiontoananchorpointatone
endthatservesasameansforconnectingothercomponentsofapersonalfallarrestsystemto
theanchorpoint.

• Provideconsultationandtrainingtodepartmentsasnecessary
• Assistdepartment(s)anddepartmentcompetentpersonindeterminingwhen,
where,andhowfallprojectionmaybeapplied.
• Assisteachdepartmentinidentifyingelevatedworkareas.
• Assistdepartmentsindeterminingwhichfallprotectivesystemwillworkbest
foreachelevatedworksituation.

GUIDELINES/RULES:
ThefollowingitemsarecoveredbytheUniversity’sFallProtectionProgram:
1. Ladders:fixed,portable,freestanding,temporary,orrollawaytype
2. ElevatingPersonalPlatforms:scaffolds,aerialplatforms,scissorlifts,forkliftmountedplatforms,buckettrucks,etc.
3. VerticalOpenings:groundlevelentryintoexcavation,trenches,holes,pits,vessels
andotherconfinedspaces.
Fallprotectionisrequiredwheneverworkisperformedinanarea6feetaboveitssurroundings.Fall
protectivesystemsmayconsistof:
1. Guardrails:Standardguardrailsconsistofatoprail,located42inchesabovethe
floorandamid-rail.Screensandmeshmaybeusedtoreplacethemid-rail,
providedtheyextendfromthetoprailtothefloor.
2. Personal Fall Arresting Systems: Components of a personal fall arresting system include a
body harness, lanyard, lifeline, connector, and an anchorage point capable of supporting at
least 5000 pounds.
3. Positioning Device Systems: Positioning device systems consist of a body belt or harness
rigged to allow work on vertical surfaces, such as a wall, with both hands free.
4. WarningLineSystems:Warninglinesystemsaremadeupoflinesorropes
installedaroundaworkareaonaroof.Theseactasabarriertopreventthose
workingontherooffromapproachingtheedges.

5. Covers:Coversarefastenedoverholesintheworkingsurfacetopreventfalls.
Ifitcanbeclearlydemonstratedthatuseofthesesystemsisimpracticableorcreatesagreater
hazard,alternativefallprotectionmethodsmaybeimplemented.
• MonitoringSystem:Monitoringsystemsmustbedevelopedinaccordancewith
OSHAregulation1926.502andmustbeapprovedbyEH&Sortheirdesignees
beforeemployeesareexposedtofallhazards.
6.0
PROCEDURES:
6.1
Ladders
Employeeswhoworkonladderswithaworkingheightof6feetormoremustbe
knowledgeableofthefollowing:
• Howtoinspectladderstoensureladderisinsafeworkingorder.
• Howtouseladdersproperly.
6.2
Personal Fall Arresting Systems (PFAS)
Employeeswhousepersonalfallarrestingsystemstocontrolpotentialfallinjuriesintheir
workareashallbeknowledgeableofthefollowing:
• Theapplicationlimitsoftheequipment.
• Theproperhook-up,anchoring,andtie-offtechniquesincludingdeterminationof
elongationanddecelerationdistances.
• Methodsofuse,inspection,andstorageofequipment.
• Personalfallarrestcomponentsincludingharnessesandlanyardsshouldbeinspected
beforeeachuseforthepresenceofmildew,wear,damageandotherimperfections.
Defectivecomponentsshallberemovedfromservice.
• Fallarrestsystemsincludingharnessesaretobeinspectedtwiceeachyearoraccording
tomanufacturer’srecommendations.Thedateofthemostcurrentsemi-annual
inspectionshallberecordedonaninspectiontagwhichshallbeattachedtotheharness.
Inaddition,separaterecordsshallbekeptandmaintainedshowingdateofpurchase,
dateswhenattachmentswererenewed,anddateswhentheentireharnessassembly
wasinspectedandbywhom.
6.3
Aerial Lifts
Employeeswhouseaerialliftsmustbeknowledgeableofthefollowing:
• Themanufacturer’soperatinginstructions.
• Pre-startinspectionofthelift.
• Inspectionoftheworkareafordangerousconditionssuchasunevensurfaces,
overheadobstructionssuchaspowerlines,orotherhazards.

Universityemployeeswhoworkonscaffoldsmustbeknowledgeableofthefollowing:

• Thecorrectprocedureforerecting,securing,maintaininganddisassemblingthefall
protectivesystemsandfallingobjectprotectionsystemsused.
• Theproperuse/constructionofthescaffold.
• Theproperhandlingofmaterialsonthescaffold.
• Themaximumloadandcarryingcapacitiesofthescaffolds.
7.0
TRAINING:
Allemployeesworkingonelevatedworksurfacesof6feetormoremustreceivetraining.All
supervisorswhoseemployeesworkat/onelevatedworksurfacesandmayberequiredto
utilizefallprotectiveequipment/systemsshallalsoreceivetraining.
Nouniversityemployeeshallworkinareasofhighfallhazardsorperformdutiesrequiringthe
useoffallprotectiondevicesuntilhe/shehascompletedtraininginfallprotection.
7.1
Employee Training:
Trainingshallbeprovidedforallemployeesworkingonjobtasksrelatedtoelevated
platforms.Trainingshallincludethefollowing:
• Classroomtraining.
• Onthejobtraining.
• Inspection,careanduseofpersonalfallprotectiveequipment
• Emergencyactionplan,incaseofafall.
• Demonstratingcompetencyinoperatingequipmentsafely.
• Recognize the hazards of falling and procedures to be followed in order to
minimize these hazards.
Employeeswillrequireretrainingunderanyofthefollowingconditions:
• Changesinworkplaceresponsibility.
• Changesinthetypesoffallprotectionsystemsorequipmenttobeused.
• Inadequaciesinanemployee’sknowledgeoffallprotectionsystems,equipmentor
observedbehaviorwhichdemonstratestheemployeehasnotobtainedtherequired
training.

Supervisor Training;
• Supervisorsshallreceivetrainingonthefollowingtopicspriortobeingassignedto
superviseemployeeswhoworkonelevatedsurfaces.
