No Refunds….. Tickets are NOT transferrable.

Want to bring a Non-CHS Guest?
Here’s what you need to do.
1. Print out the permission form for CHS Student and Non-CHS Guest.
2. Complete the form and return it to the CHS Office for approval by Nov. 3rd at 3:00 p.m.
NO EXCEPTIONS.
You must include the following with your Non-CHS guest form:
* Photocopy of guest’s 2015-2016 Student ID or Driver’s License
* Their school administrator’s approval and business card
* All required signatures (Forged signatures will result in exclusion from the dance.)
No Guest Forms will be accepted after 3:00 p.m. Nov. 3rd
No Middle School guests or guests over the age of 20.
CHS Student and Guest must check in and attend the dance together.

prior to 10:30 p.m. without parent notification.
I agree not to furnish or condone the use of illegal drugs or alcohol for my son/daughter prior to
and in transit to and from the dance.

I understand that sexually explicit dancing (“grinding,” “freaking,” “twerking”) is not permitted, and I may be
removed from the dance floor and kept in a separate room, if I engage in inappropriate dancing
(at discretion of CHS Administration).
I agree NOT to use drugs and alcohol prior to/or during the dance. I agree to abide by all school rules. I understand that unexcused absences and/or excessive tardies may render me ineligible to attend the dance. I
understand that an out-of-school suspension during the semester of the dance renders me ineligible for the
dance. Should I violate school rules at the dance, I understand that I may be suspended or expelled, and I may
lose the privilege to attend ALL the remaining school activities through June 9, 2016. I further understand that
if I appear at the dance under the influence of drugs or alcohol, I will not be admitted and will be detained until
a parent, guardian, or authorities arrive to take responsibility.
I have read this permission sheet, CHS Dance Policy, and am familiar with the rules in the student handbook. I realize that infractions of the school rules will result in consequences up to and including suspension/expulsion from
school. I also acknowledge that forged signatures will result in exclusion from the dance.
