Promotional dues and zero initiation fees are only valid through June 30, 2013. The SCC application process includes submission of completed SCC membership application, credit check, signed 2013 membership promotion agreement.
Under this promotion new SCC members will not be responsible for any additional initiation fees throughout the course of their
membership.
Active golf members must purchase their membership stock certificate in June 2015. Intermediate golf members must make a
deposit of 1/2 of their membership stock certificate in June 2015. Certificates are $4,000 and an early payment plan of $100
per month for 40 months can be established upon joining, if desired.
New members may join without a commitment through June 2015 by paying initiation fees and full dues for the membership
category under which they join.

SYLVANIA COUNTRY CLUB 2013 MEMBERSHIP AGREEMENT
This is my agreement to become a member of Sylvania Country Club (the “Club”) under the 2013 membership program, with the
following terms and conditions:



Under this promotion, I will not be charged any initiation fee for the lifetime of my membership. I agree to pay the promotional
monthly dues through June 30, 2015.
I understand that I have until June 30, 2013, to withdraw my commitment through June 30, 2015. If I choose to withdraw from
the commitment agreement, I agree to pay the appropriate initiation fee for my classification and continue at the club paying full
dues, or I will leave the club without any further commitment.
By accepting this membership agreement, I will pay the dues for my applicable membership classification and age established
from time to time by the Club through June 30, 2015 less the following monthly discounts: Active-$100; Intermediate-$70 and
Junior $37.

I also agree to pay all scheduled periodic and annual Club fees for my membership class and all proper charges to my account.
If I choose to end my membership prior to June 30, 2015, I agree to pay the remaining month’s dues, if any, and periodic and annual fees, and proper charges to my account, through June, 2015, to satisfy my commitment.
 I understand that if I should pass from one category to another because of a birthday, and age up to the next class (e.g. Intermediate to Active), I will still continue to enjoy the promotional dues discounts as listed for my new classification.
 I understand that I may transfer from a Social to a Golf membership during the commitment period without penalty; however, I
may not transfer from a Golf membership to a Social membership during the commitment period.
 Should an Operational Assessment be passed during the commitment period ending June 30, 2015 any new member under this
commitment will not be required to pay.
 Should a Capital Improvement Assessment be passed during the commitment period ending June 30, 2015 any new member under this commitment will have the option to pay and remain a member or opt out of paying the assessment and leave the Club
without further charge.
 I understand Active Golf members must purchase the $4,000 membership stock certificate on or before June, 2015. Intermediate
members will be required to begin purchase of half of the stock certificate deposit ($2,000), the remainder of which will be due
upon transfer to Active membership status. New members may opt to begin payment on their stock purchases at the time of joining in the amount of $100 per month for 40 months. Should the new member leave the Club before final payment those amounts
would be forfeited.
 As an Active or Intermediate golf member, I agree to pay the Club’s required annual golf fees for the Mill River Program (2012$125), Locker Fee (2012-$150), and Bag Storage (2012-$100). And such other fees established from time to time by the Club.
 I understand as an SCC member (golf & social) I am required to pay the fee charged for the Club’s annual fireworks display.
(2012 charge was $45).
 I understand the membership application process and agree to abide by all the Club Rules and Code of Regulations, except as
modified by this Agreement.
