n Seems physically fragile or unsteady when
n Appears confused or forgetful

n Has fallen more than once, with or without
injury
n Is unable to clearly explain things or describe
needs
n Has lost or gained a significant amount of
weight
n Shows mood or personality changes
n Is increasingly isolated and refuses to go out
n Doesn’t take medications as prescribed
n Often complains of feeling fatigued
n Has dirty clothes, hair or bedding
n Refuses to go to the doctor or dentist
n Is newly suspicious of previously trusted
relatives
n Has stacks of bills, paperwork or unopened mail
around the home
n Has experienced deterioration of relationships
with others
n Has body odor or odors in the home
n Has difficulty seeing or hearing well enough to
communicate properly
n Doesn’t remember asking about the same thing
repeatedly
Experiences other significant life changes:
n Has unexplained injuries or bruises
n Has many alcoholic beverage bottles in the trash
n Has not adequately maintained the home
n Has suffered the loss of a spouse, child, partner
or pet
n Has recently lost the right or the ability to drive
n Has had significant others move away
n Is no longer involved with friends, neighbors or
community
