Applications are invited from state and private institutions that wish to register for the supply of
following supplies and services for the year 2017 for the Head Office of the Department of
Agriculture, peradeniya and all Regional offices of the Department that function under all
Administrative Districts.

(a)
Application forms issued by this department for the registration of suppliers for year 2017
should be duly perfected and sent by registered post to receive before 15.12.2016 and the
application for services under A should be addressed to 'Director General of Agriculture.
Department of Agriculture. No. 25, Old Galaha Road, Peradeniya, and the applications for
services under 8 should be addressed to "Chief Engineer, Chief Engineer's Office, Department
of Agriculture. Peradeniya. The caption "Registration of Suppliers -2017" should be mentioned
on the left hand top corner of the envelope containing the application.
(b)
Application forms may be obtained from the following places. free of charge, during office
hours on working days up to 22.12.2016.
Places where application forms may be obtained :

Application for supplies under A may be obtained from the o/2 branch of the Administration
Division, Department of Agriculture. No. 26.Old Galaha Road.Peradeniya.

Applications for supply of services under B may be obtained from the Engineering Division of
the Department of Agriculture at the atore said address.
(e)
Separate applications should be submitted for each category. A registration fee of Rs. 500/should be paid for each application and it may either be credited to the People's bank Account
No. 057100139027201 or Bank of Ceylon Account No. 0007042565 in favour of Director
General of Agriculture- and a copy of the receipt obtained from the bank should be attached to
the application. A payment also be made in cash to the shroff of this department and the rece'pt
obtained should be attached to the application.
(d)
Registration wig be considered only for items or services mentioned in the Business
Registration Certificate and photocopy of the Business Registration Certificate should be
attached to the application
(e)
Separate applications should be submitted for category A - Supplies and category B - Services.
Institutions that apply for more than one item may prepare the necessary applications as
mentioned above and may submit in a single envelope. But the registration fee as mentioned
under C above for each item should accompany each application along with a copy of the
business registration certificate.
(f)
Registration of the registered suppliers who failed to submit quotations when requested and of
those who fail to supply items or services within the stipulated time period will be cancelled,
without any notice.
(g)
Late applications will be rejected without any notice.

Registration of Contractors for civil Construction and Maintenance
Works in Year 2017
Applications are invited province-wise and as per the contract value for registration as
contractors for Civil Construction, Maintenance and repair works of the Department of Agriculture for
year 2017.
Each contractor may submit applications for any two provinces and for both contract values
under that province and those applications may be submitted under single cover. Registration for each
category for each province is given below.

Relevant applications for bulldog works will be issued to only contractors who are presently registered
with the Institute for Construction Training and Development (ICTAD). Further. Contractors who seek
registration works from Rs. 1,000.000/= up to Rs. 2.000.000/. should hold the membership with the
National Construction Association of Sri Lanka (NCASL). Application forms may be obtained free of
charge from the Chief Engineer. Department of Agriculture. P.O. Box 08. Peradeniya upon producing
the necessary documents to prove the above requirements. Applications should be submitted to the
above address to receive before 15.12.2016 and the caption "Registration of Contractors for Civil
Construction and Maintenance Works - 2017" should be mentioned on the left-hand top corner of the
envelope containing the application. Incomplete applications that are not submitted as per the relevant
applications will be rejected. It is mandatory that the ocpy of the Registration Certificate issued by the
Institute for Construction Training and Development (ICTAD) and the contractors who seek
registration for contracts from Rs. 1.000,000As up to Rs. 2.000.000/- should also enclose a copy of the
membership certificate with the National Construction Association of Sri Lanka.
