Vacancy Exception Process

AUDIENCE: Residential Habilitation Service Providers that signed the Addendum to Provider
Agreement for the Settlement Agreement*
PURPOSE:
This communication includes information for providers regarding the procedures
related to the residential habilitation vacancy factor for both FY 15-16 and 16-17.
There has been no change except the replacement of the previously sent Vacancy
Exception Request Form_FY1517 with the Vacancy Exception Request
Form_FY1517 included with this announcement.
DISCUSSION: ODP made the decision to continue the Vacancy Exception Process for FY 15-16
and FY 16-17, similar to the process for FY 14-15. As you are aware, in order to submit a
request for an exception to the vacancy factor for FY 15-16 and FY 16-17, the provider shall
complete the Vacancy Exception Request Form_FY1517 included with this announcement

contact your Regional Fiscal Officer (RFO) for the expanded version of the form. The provider
must email the Excel template to the Regional Program Manager and Regional Fiscal Officer by
SEPTEMBER 30, 2016 for FY 15-16 and SEPTEMBER 29, 2017 for FY 16-17, if your agency
completed billing for the fiscal year. If you agency has not completed billing by the due date
and you anticipate the need to submit, please contact your RFO.
*Please refer to Informational Memo 003-13 if you are a residential provider that did not sign
the Addendum to Provider Agreement for the Settlement Agreement and wish to submit a
vacancy exception request.
