I hereby authorize the staff of the Eagle-Elite Basketball Camp to
act in accordance with their best judgment in any emergency
requiring any medical attention and I hereby waive and release the
camp and its employees from any and all liability for injuries
incurred while at camp. I have no knowledge of any impairment
that would be affected by the above named camper’s participation
in the camp program as outlined in this brochure. In signing this
application, I acknowledge that this camp is not owned or operated by the Middletown Board of Education. The MTBOE, its
employees and agents are not responsible for, and shall be held
harmless against, any claim of injury or damages, including medical
costs, in the event of an injury to the above individual sustained
while participating in this camp.

OUR DRILLS ARE D ESIGNED TO ENCOURAGE GROWTH IN ALL ASPECTS OF
THE GAME. THE GOAL IS TO EQUIP

SKILLS TO HELP FOSTER THEIR SUCCESS
IN SCHOOL, CYO, AAU, MIDMONMOUTH, AND RECREATIONAL
LEAGUES. THE SPRING IS A GREAT TIME
