It's Time to Register for Kindergarten!
What Should I Know?
If your child is 5 (or almost 5) it’s time to sign up for Kindergarten! Contact your school directly for
birth date requirements and any other questions you may have.
Visit www.uwlc.net/SuccessBy6. There you will find tools to help you and your child prepare for
success in school as well as links to all area school districts
What Should I Bring?
Because each school has different requirements, it is very important to check with your local school
district for the documentation that is required for registration. You will need the following items to register
your child for public school:
Birth Certificate
Social Security Card
Current immunization record
Health History Form completed by parent/guardian
(obtain from the school at the time of registration.)
Proof of Residency such as a driver’s license or rental/mortgage agreement*

* Each school district determines what type and how many
proof of residency documents are needed to register your
child. It is important to contact your local district for this
information.
