WAIT LIST POLICY FOR FUNDED IUI/IVF CYCLES Policy: Page | 1 At Anova Fertility and Reproductive Health, our intent is to manage a wait list for OHIP Funded IUI/IVF treatment as fairly and transparently as possible. Our policy for handling our wait list is that it is a "first come, first served" wait list for patients of Anova Fertility & Reproductive Health. Our wait list is maintained in our secure Electronic Medical Records system (EMR) which has an audit log to show all changes made to it and includes tracking of dates when the patient is added, the target date of treatment and their current status. Employees have been trained on managing this system appropriately and the wait list will be closely monitored to verify its accuracy. Any concerns about the wait list policy may be addressed to: Teresa Reid Financial Service Manager (416) 225‐4440 ext. 336 Procedure: How does a person get on the wait list? People who want to become patients of the clinic must get a referral and have a consultation with one of our Fertility specialists. Once the Fertility Specialist has determined IVF is an appropriate treatment and the patient has indicated they are "ready for care", then the person can get added to the wait list. How long is the wait list? We cannot predict exactly when a patient will be called for their IVF treatment start at this point. We will contact patients with their anticipated start dates for cycles, however, this is only an estimate as many factors can influence our ability to provide the number of IVF cycles per month that we hope to do. We cannot guarantee that patients farther along on the wait list will receive funded IVF treatment in the 2016/2017 year as we have been given a specific budget allocation by the Ministry of Health that once reached means we cannot provide any additional funded IVF cycles for that fiscal year (April 1st ‐ March 31st). These patients may need to wait until the following fiscal year before they can receive care. We will inform patients as soon as we believe this might be a possibility. What does "Ready for Care" mean? This is a wait list for patients who are currently "ready for care". This means the patient wants to go through an IVF cycle as soon as possible and there are no restrictions preventing them from undergoing that care (e.g.: don't require surgery, have the time to come to the Clinic regularly for care). If a patient is NOT "Ready for Care" when called, their name will need to be taken out of their current position on the Page | 2 wait list and added to the bottom of the list. We cannot guarantee a funded IVF cycle to those who choose to waive their initial spot on the wait list. Removal from the Wait List A patient will be removed from the Wait List as follows. The purpose of this policy is to provide those who are eligible for a funded IVF cycle and are "ready for care" the quickest possible access to care: A patient's funded cycle is considered complete according to the Ministry of Health guidelines A patient is pregnant and graduated from our Clinic when called to start their funded IVF cycle A patient has been contacted but did not respond to two (2) attempts to contact them (using the phone numbers the patient has provided to the Clinic). Both attempts to contact the patient will be recorded in our audited EMR by the person who made the phone calls for tracking. The patient has reached the age of 43 years of age without being validated for their funded IVF cycle Exceptions None. We will not bump patients to the top of the list for any circumstances including for those who are 42 years old and about to age out of the Funded IVF program eligibility. *patients will also be given notice in regards to this policy.
