Automatic Tank Gauge monthly reports must be completed, at least last twelve (12) months must be submitted to
IDEM. The Automatic Tank Gauge was in a locked room and we were denied access to it.
Annual Line Tightness Test and Line Leak Detection testing must be completed and reports sent to IDEM.
All drums located in back of store, which contain hazardous waste, must be removed and properly disposed of within
thirty (30) days.
All sump pits need pumped out and. All spill buckets need pumped out and all debris needs to be removed, also a
hand pump must be on site. Dispenser catch basins need to be cleaned, there were several used filters in bottom of
basins that must be properly disposed of. All above items must have work order or invoice emailed to IDEM.
Operator Certification must be completed. No B or C Operator Certification were available. Complete and submit B or
C Operator Certificates to IDEM.
Resubmit and complete Notification Forms and Financial Responsibility Form.
Note; Kerosene tank is currently shown as "in use" but appears to be temporarily "out of use".

***FOLLOW UP INSPECTION 5-18-16***
Facility must correct or provide the following to IDEM within thirty (30) days:
1. Monthly leak detection must be completed. The facility's automatic tank gauge (Veeder Root ATG) is not
operational. The facility states they are manually sticking the UST's and recording the results but have no third party
analysis results;
2. Multiple 55 gallon drums of suspected industrial waste are stored on the property. The materials must be properly
characterized and disposed of. Disposal verification manifests must be provided to IDEM;
3. Class B & C operator training must be completed and certificates on file at the facility; and,
4. Notification form #45223, certificate of financial responsibility (COFR) form, and certificate of financial assurance
(COFA) form must be updated and submitted to IDEM.
Send documentation/invoice/work order to IDEM within thirty (30) days.

The automatic tank gauge (ATG) is not operating. Statistical inventory reconciliation (SIR) results are not being
recorded properly. No form of leak detection is being completed.
There are eleven (11) fifty-five gallon drums with contaminated product being stored behind the facility. There are
three (3) fifty-five gallon drums with contaminated product being stored inside the facility.
