THIS ADDENDUM No. 1 (RATE PROPOSAL FORM) SHALL BE UTILIZED FOR
SUBMITTING COST AND PRICING. IT MUST BE SIGNED AS ACKNOWLEDGED AND
SUBMITTED WITH PROPOSAL.

(Maximum Points: 60, Weight: 25%)
Costs and pricing provided shall be fixed and include all delivery and associated fees; and where
applicable, based on exacting indexes and benchmarks. Do not include government mandated taxes.
The COUNTY reserves the right to conduct price calculations based on historical quantities provided in
the RFP and consider location of commercial stations, in proximity to the areas within the County that the
fleet routinely travels, in order to conduct a comprehensive review.

will be updated on a daily bases throughout the life of any awarded contract.
PROPOSALS THAT DO NOT INCLUDE A COPY OF THE OPIS REPORT WILL NOT BE
CONSIDERED.
