SCHOLARSHIP APPLICATION
Charles R. Drew University (CDU) offers a variety of institutional scholarships. Eligibility requirements of each scholarship are
available at http://www.cdrewu.edu/scholarships. Scholarships are available to undergraduate and graduate students. Students are
encouraged to apply for CDU scholarships every year.
Complete and submit this application to the Office of Financial Aid and Scholarships by no later than the published deadline of March
10, 2017 at 6pm, together with a resume/CV and a 2 page personal statement that describes your personal connection to the CDU
mission and your strengths in areas of Community Service, Leadership, Scholarship, and Research. Only those applications received
via email to [email protected] by the published deadline will be considered. Applications received after the published
deadline will not be considered.
To be eligible for any CDU Scholarships, candidates must be registered in a minimum of 6 credits as of the close of the add/drop date
of the semester to which the award is to be applied. In addition, candidates must meet CDU’s satisfactory academic progress policy
and not be in default with any federal aid programs during the semester/s to which the award is to be applied. Applicants are
responsible for verifying that all required documentation is complete and submitted by the deadline. Incomplete applications will not
be considered.
For scholarships that require recommendations, recommendation forms must be submitted on the CDU recommendation form and
delivered to the Office of Financial Aid & Scholarships in a sealed envelope, with the signature of the recommender across the sealed
envelope flap. Unsealed recommendations will not be accepted. Recommendations may also be emailed by the recommender to
[email protected]. Students applying to scholarships requiring recommendation letters are encouraged to request
recommendation letters at least 15 days prior to the deadline.

Biography:
☐ Graduate Education Scholarship
The Graduate Education Scholarship is designed to help close the ‘unmet need’ gap which is often a barrier to success for academically
qualified graduate students. This scholarship is open to graduate students only in the College of Science and Health (COSH), College of
Medicine (COM) and Mervyn M. Dymally School of Nursing (MMDSON). Funding for this scholarship is provided by the U.S.
Department of Education Title III Program.
Additional Criteria/Documentation

Consideration will be given to students who demonstrate unmet need in accordance with federal financial aid guidelines.
• Candidates must have had completed the Free Application for Federal Student Aid (FAFSA) by the time of this scholarship
application.

The Office of Financial Aid and Scholarships will request the FAFSA from the David Geffen School of Medicine for
College of Medicine (COM) applicants.
☐ Haynes Family
The Haynes Family Scholarship is open to CDU students in the College of Science and Health (COSH), College of Medicine (COM) and
Mervyn M. Dymally School of Nursing (MMDSON).

The William Hearst Scholarship is available for full-time students in the College of Science and Health (COSH), with an academic GPA
standing of 3.0 or higher, who have demonstrated outstanding leadership in a student organization.

3 letters of recommendation (i.e. from the candidate’s academic program, university or dean)

The AT&T and Matching Fund scholarship is available for full-time students in the College of Science and Health (COSH), with an
academic GPA standing of 3.0 or higher, who have demonstrated outstanding leadership in a student organization.

☐ Primary Care Scholarship Program (funded by Kaiser)
The Primary Care Scholarship Program is available for Graduate students in the Mervyn M. Dymally School of Nursing (MMDSON),
College of Medicine (COM), and Physician Assistant students in the College of Science and Health (COSH).

• Include a separate statement that addresses career goals and commitment to practice in a community clinic and/or
medically underserved area after degree completion.

If awarded, student must acknowledge and agree to the following:
o 15% of their scholarship will be withheld annually each year that a scholarship is received as an incentive to seek
and secure employment in a community clinic or medically underserved area
o Provide evidence of employment in a community clinic or medically underserved area and/or evidence of
participation in a primary care residency program within 3 months of graduation.
