Thank you for choosing Ayres Family Cremation. We are locally-owned and operated, serving all of Humboldt County. Our family, helping our neighbors with affordable cremation and
direct burial services.
The goods and services shown in this packet are those we can provide to our customers. You
may choose only the items you desire. However, any arrangements you select will include a
charge for our basic services and overhead. If legal or other requirements mean you must buy
any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the goods and services you’ve selected.
Preneed Disclosure
Prior to drafting any contract for goods or services, the responsible party or the decedent’s
survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed
agreement in the possession of the funeral establishment that has been signed and paid for, in
full or in part, by or on behalf of the decedent.
Terms of Payment
Payment in full is expected by the date of services or final disposition. Ayres Family Cremation
accepts cash, debit cards, checks, and credit cards (Visa, Master Card and American Express).
We only accept insurance assignments of policies sold by Ayres Family Cremation or our
agents.

FUNERAL SERVICE PACKAGES
Direct Cremation Package $965 - $975
$965 direct cremation with cremation container provided by the purchaser
$975 direct cremation with container provided by the funeral home
If you want to arrange a direct cremation, you can use an alternative container.
Alternative containers encase the body and can be made of material like fiberboard or composition
materials (with or without an outside covering). The containers we provide are cardboard or plywood
bottom with a cardboard top.
The Package includes:
• Basic Services of the Funeral Director and Staff
• Removal from the place of death 24 hours/day, 7 days a week (within Humboldt County)
• Climate-controlled holding (refrigeration) up to 10 days
• Brief viewing and/or present at cremation, (10 persons max.) (if requested)
• All necessary paperwork and permits
• Cremation (additional charge for persons over 300 lbs.)
• Cardboard cremation container (required by CA state law)
• Temporary plastic urn
• Storage of cremated remains up to 30 days
• Sales Tax

• Basic Services of Funeral Director and Staff

The charge for our basic services and overhead will be added to the total cost of arrangements you
select. (Note: These services are included in our direct cremations and direct burials). This charge
includes, but is not limited to availability of staff 24 hours per day, staff and facilities to respond to
initial requests for services, consultation with the responsible party to determine the services desired,
coordinating service plans with other parties involved in the final disposition of the deceased, filing of
local death certificate and obtaining local permits with the appropriate department (there may be an
additional fee charged by the local jurisdiction).

The terms “protective” and “non-protective”, which may be found in some printed material provided by the
manufacturer, were chosen by the manufacturer and do not constitute or imply a representation or warranty by
the funeral home. Although certain caskets offered for sale by this funeral establishment may be of better
quality construction and comprised of more durable material than others, neither this funeral establishment nor
any of its employees represents or implies that any casket will be airtight or watertight or will provide long-­term
preservation of human remains.
The only warrenty on the casket or any merchandise sold in connection with this service is the express written warrenty, if
any, granted by the manufacturer. FUNERAL HOME MAKES NO WARRANTY, EXPRESS OR IMPLIED, INCLUDING AN IMPLIED
WARRANTY OF MERCHANTABLITY AND AN IMPLIED WARRANTY OF FITNESS FOR A PARTICULAR PURPOSE, WITH RESPECT TO
THE CASKET AND/OR ANY MERCHANDISE SOLD.
There is no scientific or other evidence that any casket with a sealing device will preserve human remains.
