DSL Manual Loop Qualification Inquiry Process
Overview
A DSL Manual Loop Qualification Inquiry Form has been developed to assist Wholesale Customers
with Loop Qualification issues.
Description
Wholesale Customers who choose not to use FairPoint’s pre-ordering interfaces, or are experiencing
trouble when obtaining loop makeup (LMU) data, can request a Manual Loop Report (MLR) from the
LSC.
Wholesale Customer Responsibilities Related to Submitting the FairPoint DSL Loop
Qualification Inquiry Form
The Wholesale Customers must populate the following items on the “DSL Manual Loop
Qualification Inquiry Form” located in the Manual Process Forms section of Wholesale
Customer Online (VFO) Pre-Order:
Loop Qual Submitted by Working Telephone Number (WTN) or Address
Note: If the CLEC has submitted a Loop Qual and received no response or invalid
data, the Wholesale Customer must indicate if the Loop Qual was submitted by the
Address or WTN.
If the problem is in relation to Address or WTN, then the following fields must be
populated:
o Loop Qual Submitted, No Response
o Loop Qual Submitted, Data Provided Invalid
o Unable to Retrieve LMU/Unable to Submit Loop Qual

If using Address for validation to submit MLR:
o Enter Complete End User address at which the service is to be delivered
exactly as Wholesale Customer is inputting into the VFO loop qual system
(e.g. street number and name), any supplemental information (e.g. APT,
AHN,), City, State, Zip. Also provide WTN working at the address if available.
If using WTN for validation to submit Loop Qual:
o Enter WTN working at End User address at which the service is to be
delivered.
Note: If possible, please also provide the end user’s address even when
validating by WTN.
BTN (new column added to the form). In the event the Wholesale Customer is
attempting to obtain Loop Make Up information for a WTN that is NOT the main billing
TN, please provide the main BTN for the account.
If the End User at the Address or WTN being used for validation has recently moved, or is about
to move locations, be sure to populate the End User’s address at which the service is to be
delivered, or the WTN at which the service is to be delivered on the spreadsheet. This will
eliminate the risk of retrieving loop make-up information for a location in which the End User is
no longer residing in, or will soon be vacating.
