Date Stamp 2014-2015 Parent Tax Verification Worksheet (Version A) 5PTVWA We need to verify your parent(s) 2013 income information reported on your FAFSA. If taxes were filed, the best way to verify this is by using the IRS Data Retrieval Tool (IRS DRT) that is part of FAFSA on the web. (Follow instructions on next page). In most cases, no further documentation is needed to verify 2013 income information that was transferred into the student’s FAFSA using the IRS DRT if that information was not changed. What you should do: 1. Complete and sign this worksheet with your parent (s). Do not leave any answers blank. If blanks occur, the form may need to be returned causing delays. The verification process can take 6 weeks or more to complete. Missing documents and incomplete paperwork will cause delays. 2. Submit this worksheet to the Financial Aid Office within 30 days.
