Client Files Therapeutic Family Services (TFS) offers individual counselling to children, young people and adults who have experienced abuse and/ or trauma and provide out reach services to children and families who live in supported housing programmes. TFS programmes include: All TFS clinicians are required to keep a record of each session attended and any other relevant information regarding your treatment. This information is kept in your personal client file which is held in a locked secure place within the TFS office you attend. Trauma Assessment and Treatment Service; Confidentiality At any time you may request: To have access to your file by writing to Parkerville Children and Youth Child Sexual Abuse Therapeutic Service; Care’s Chief Executive; Support and Counselling Service. To know where your personal information is being held and how In the following specific circumstances, however, your clinician may be required to disclose information without your consent: long it will be held for; If there is a risk of harm to yourself or others; or To correct in writing any information If a court of law subpoenas TFS to supply information about you. you may feel is inaccurate; Everything you discuss with your clinician is confidential and will not be disclosed without your written consent. Your clinician/ case manager has an ethical responsibility to maintain confidentiality. To know which organisations have/ will be given your personal information. In the event of these circumstances, your clinician will make every effort to discuss with you any disclosure of information prior to releasing the information.
