The Community Placement Team Leader is also responsible for the provision of support to and advocacy for Community Carers, recruitment, assessment and review of Community Carers and Holiday Hosts and the training and development needs of all Foster Carers within the Community Placement Programme. Duties and Responsibilities Key items only (do not list every task) 1. 2. 3. 4. Case Management 1.1 Case manages children who have been placed by the Organisation in out of home care with appropriately assessed carers within the community. 1.2 Provide appropriate support and assistance to deal with problems of everyday life, to children who may display a number of behaviours associated with abuse/neglect, trauma, attachment, separation, grief and loss. 1.3 Ensure assessment, planning and intervention processes occur for each child within the service as directed by Therapeutic Care Planning processes. 1.4 Coordination of a Therapeutic Care Team which works collectively to meet the dimensions of wellbeing needs of each child within the programme. Recruitment, Assessment, Review and Development 2.1 Responsible for the recruitment and assessment of Community, Respite and Holiday Host carers. 2.2 Ensures that all carers within the Community Care Programme are reviewed on an annual basis. 2.3 Works to ensure appropriate training and development opportunities are provided to all Community Carers, Holiday Hosts and Respite Carers. Quality 3.1 Conduct ongoing research, review and assessment of the Community Placement Model. 3.2 Represents the Therapeutic Care Programme and Community Placement Programme on sector and agency forums relevant to out of home care service provision. 3.3 Ensures implementation of service standards as outlined within agency service agreements and the Better Care, Better Services standards in out of home care. Administrative Duties 4.1 Maintain appropriate record keeping and reporting requirements, in line with established processes. 4.2 Collect, maintain and circulate data relevant to agency service standards and Agreements. 4.3 Participate in on-going supervision with the Director Children’s Services on a minimum fortnightly basis. 4.4. Participate in after-hours/on-call service on a rostered basis and receives an additional week’s annual leave per annum. Other 5.1 Understand, support and promote the Mission and Values at Parkerville Children and Youth Care. 5.2 Commitment to and participation in the Organisation’s System Improvement Processes. 5.3 Other duties as requested by the Chief Executive or Director Children’s Services. Selection Criteria Essential and desirable requirements to meet duties and responsibilities (incl. qualifications) The proven ability to work within and contribute to the mission and values of Parkerville Children and Youth Care underpins the position requirements outlined below and is an essential selection criterion for all Parkerville positions: 1. Qualifications in Social Work, Psychology or Nursing. 2. Significant Experience in case management of children affected by abuse and trauma. 3. Capacity to work as a member of the Parkerville Children and Youth Care multi-disciplinary team. 4. A high degree of initiative and ability to work autonomously. 5. Understanding of the complexities of abuse and trauma for children and an understanding of the issues and challenges facing these children. 6. Excellent interpersonal and communication skills with a diverse range of professional and nonprofessional staff. 7. A “C” class drivers’ licence. Desirable 8. Knowledge of any legislation relevant to this position (e.g. Child and Family Act). 9. Commitment to personal development and continuous improvement of professional skills.
