うつ病を有するドライアイ症例の臨床的特徴について

Vehof らは，うつ病に伴うドライアイの臨床的特徴について興
味深いデータを報告した。本研究はうつ病を有するドライアイ
患者の自覚的・他覚的所見の間に差があるかどうかを明確
にすることを目的としている。Groningen longitudinal sicca
study（GLOSSY）
の対象者より662 名のドライアイ患者のう
つ病の有無と涙液機能，眼表面所見が検討されている。自
己問診票を用いて，過去にうつ病と診断された，またはうつ
病の治療を受けていた患者を分類し，The ocular surface
の自覚症状スコアを用いてドライア
disease index©（OSDI©）
イ症状の有無を評価した。本研究では，
ドライアイと診断され
た患者の 11.2 ％が過去にうつ病の診断を有していた，または
うつ病の治療を受けていたと報告されている。うつ病患者の
OSDI© の自覚症状スコアはうつ病のない群と比較して有意に
高く，涙液浸透圧，結膜充血スコア，シルマーテスト値，矯正
視力値，角結膜上皮障害スコア，涙液層破壊時間，Mucus
ferning test スコアにおいては，うつ病との有意な関連が認
められないと報告された。本研究ではうつ病の既往歴を有
する者では，その他の臨床所見に差がないのにも関わらず，
OSDI© によるドライアイの自覚症状が強くなることが示唆され
