Dual Enrollment Student CHECKLIST _____1. Have a 3.0 GPA (except for SLS courses which require a 2.5 weighted) _____2. Present proof of college‐ready scores* on the FCAT, ACT, SAT, or PERT (only [at MDC] 2 PERT attempts allowed while in 9th and 10th grade and two new PERT attempts while in 11th and 12thgrade) _____3. Go to high school advisor/guidance counselor and obtain a Dual Enrollment Approval Form A. Complete student and parent/legal guardian information (signature required) B. Select MDC Course, reference #, course title, and H.S. requirement met C. Obtain signature of High school counselor and principal D. Apply the High school seal _____4. Students must submit a paper Miami Dade College Admission application (DO NOT apply online) together with a Dual Enrollment approval form, at least 30 days in advance of the requested term of enrollment. Take these forms to room 1121. *Are my scores college‐ready? See reverse side or refer to the PERT Placement Flyer. *If you have no scores or scores that are NOT college‐ready, you may take the PERT at MDC, but you must present the PERT Placement Test Referral Form which must be filled out and signed by a high school official. Provide Dual Enrollment Form Verification for Students Registered with M‐DCPS FL Home Education Program. B. Submit High School Graduation Plan that clearly delineates courses taken through home school and courses to be taken at MDC and through home school. C. Have your parent/legal guardian complete the Home Education Dual Enrollment Articulation Agreement. D. Complete Dual Enrollment Approval Form. _____4. FCAT: Score report must be submitted to MDC in a sealed envelope with the stamp/seal of the high school. ACT and SAT: Scores must be submitted from the College Board directly to MDC (electronically). PERT: Scores can be located on a shared PERT repository site online.
