User Manual for Agami SIM distribution process through SMS & Online The registration process is only applicable for GPA5 holders whose SSC passing year is 2010, 2011 and 2012 and GPA-5 holders having passing 2013 or 2014 who didnвЂ™t withdraw Agami SIM yet. The date and time of SIM collection will be notified later after completion of registration process via SMS, TBL Website (www.teletalk.com.bd) and TBL facebook (www.facebook.com/yourTeletalk) Steps for GPA-5 SIM collection System: Step-1: Type a SMS in Teletalk mobile as per below instruction and send it to 16222 SMS format: GPA5<space>Board<space>Roll<space>Year<space>Mobile no. Example: GPA5 GPA5 DHAKA DHA 12345 2013 015******** or 12345 2013 017******** Step-2: Subscriber will get the authenticity which is as follows: Applicant name and a GPA-5 registration no. Example: XXXX 1********* Step-3: Visit the web panel http://gpa5.teletalk.com.bd and fill out the required field (GPA-5 Registration no. will be found from SMS). Student must need to indicate his/her chosen customer care in the web panel and Teletalk will supply the SIM only from that mentioned customer care. When the student will go to Customer Care Center for collecting the SIM, he/she need to provide his/her/ guardians voter ID, two copies passport size photograph etc. Below is the screenshot of URL http://gpa5.teletalk.com.bd After submission of above form user will be given an online User Registration Form. Users have to fill up all the information and after submission of the form user will be get the option to print/download the completed form which will be needed [printed copy] in the time of collecting SIM from pre-selected Teletalk customer care center. It will be also required to fill up hard copy of User Registration Form while collecting SIM from Teletalk Customer care center. It will be convenient for students if they download a User Registration Form from this link http://gpa5.teletalk.com.bd/public/saf_form.pdf and fill it up with necessary signature and then bring it along with other forms and documents in the time of collecting SIM. Step-4: Finally he/she will get a PDF version of the filled out form which he/she can either save or print out. Step-5: Go to your chosen customer care (as on your registration form).
