医療・医薬を取り巻く環境はこの数年で急激に変化をしている。

日本の医療機関におけるインフルエンザワクチンのプレフィルドシリンジ製剤の市場普及の
現状と課題に関する調査
講演概要
本講は、日本の医療機関におけるインフルエンザワクチンの接種に用いるプレフィルドシリン
ジ製剤（以下プレフィル製剤）の普及の現状と課題について明らかすることを目的としている。

調査の結果、医療機関におけるプレフィル製剤の普及率は、新型インフルエンザが流行した
2009 年から急速に高まっている傾向がみられた。
また、プレフィル製剤の導入は財政面にゆとりがないにも関わらず産婦人科で高かった。これ
は、厚労省からの通達を順守しての導入であることが窺える。
プレフィル製剤への価格受容性は診療科により違いがみられるものの導入率と価格受容性
には関連性がみられた。このことから更なる普及には医療機関が受容可能な納入価での提
供の必要が示唆された。

Title: Quality Control for high quality plastic delivery systems for pharmaceutical drug product
要旨
近年、医薬品一次包装としてのガラス製容器の問題点がクローズアップされる中、樹脂製容
器が今迄以上に注目され始め、樹脂性シリンジの重要性も高まっている。
今回、薬剤の有効性を最大限に引き出し、シリンジとしての機能性・安全性を有する樹脂製シ
リンジについて特性や品質保証の観点から紹介すると共に、今後の傾向などについても言及
する。
Abstract:
Recent global regulations on particles and delamination of glass containers in scrutiny seem to have
added to the challenges of packaging drug products in injectable drug containers. Pharmaceutical drug
products including biologics require a high quality packaging system to maintain drug stability, and to
reduce aggregation and delamination. As a result of various market needs, high quality plastic-based
packaging systems have been developed and is being used globally. The discussion will focus on quality
attributes of plastic prefillable syringes and cartridge-based systems, as well as the market trend.

演者紹介
田中拓也
1997 年、群馬大学大学院にて物質工学(化学)の博士(工学)号を取得し、同年、大協精工に入
社。研究開発部門所属し、主にゴム製品および樹脂製品の表面改質の研究に約 7 年間従事。
2004 年、ゴム部門の品質保証部にて品質保証業務に従事すると共に、滅菌済みゴム栓の洗
浄工程における製造用水についてのバリデーション業務に携わる。2006 年以降、上記研究開
発業務やゴム部門の品質保証業務の全般的な知識を活かし、シリンジやバイアルなどの樹
脂部門の品質保証業務に従事している。
間山健司
1995 年、群馬大学工学部にて応用化学科の学位を取得し、同年、大協精工に入社。品質管
理部で一連の検査業務に従事した後、品質保証部所属として 2004 年からゴム栓の洗浄工程
バリデーションに携わる。2005 年から品質試験法に関するバリデーション業務を担当し、2010
年からは CZ 製品の電子線滅菌バリデーションに関わる。2012 年現在はゴム栓の蒸気滅菌工
程の開発を担当している。

He studied mechanical engineering (MSc ETH ME: Master of Science at Eidgenössische Technische
Hochschule Zürich in Mechanical Engineering) and management (MBA BFH: Master of Business
Administration at BernerFachhochschule, that is Bern’s Advanced Technical College) and has worked in
engineering, project management and sales of technical products, and since 2005 in the field of medical
devices.

日本国内におけるプレフィルドシリンジは、膝関節症薬や比較的容量が多い X 線造影剤等
での実用化に始まり、小容量のヘパリン生食等で大幅に採用され、近年ではワクチン製剤に
も汎用されるようになっている。本講演では、小容量から大容量に至る様々な容量のプレフィ
ルドシリンジについて、素材選定や品質評価における留意点、ユーザビリティーを考慮した設
計上のポイントについて解説する。また、製造フローの事例やバリデーションの概要について

概要：
ICH-Q6A で提案されたパラメトリックリリース（PR）は、近年では ICH-Q トリオでの品質保証のあ
り方のなかで、リアルタイムリリース（RTR）の一部として議論されている。
無菌医薬品製造における PR の留意点として、PIC/S の GMP ガイドラインアネックス１７の記
載内容について、技術的な例を用いて解説したい。
