① 在留資格認定証明書交付申請書申請人等作成用１・２、扶養者等作成用１ R「家族滞在」
② 結婚証明書（配偶者）又は出生証明書（子ども）
（中国の場合公証書）
③ 生活費等に関する立証資料

奨学金証明書（扶養者のもの）
Ｂ
アルバイト先の在職証明、過去３ヶ月分程度の給与明細（扶養者のもの）
Ｃ
通帳のコピー、残高証明書（扶養者のもの）
④ 扶養者の在学証明書
⑤ 扶養者の在留カード（表・裏）コピー
⑥ 申請者のパスポートのコピー（顔写真のページ）
⑦ 写真１枚（40 ㎜×30 ㎜
申請書に貼付用）
⑧ ３９２円分の切手
⑨ 封筒（国際課を通して申請する場合は必要なし）
留意点
１．入国管理局の審査の結果、上記以外の資料を要求されることがある。
２．在留資格認定証明書が発行されても、日本領事館へのビザ申請の際には追加書類を要求されることがある。
Items for application for Certificate of Eligibility (Dependent)
1. Application forms For applicant part1, part 2 and For supporter part 1 R “Dependent”
2. Marriage certificate ( for a spouse) or birth certificate (for children)
3. Notification of financial background
A. The supporter’s certificate of scholarship
B. Certificate that the supporter is enrolled at his/her office, notification of the supporter’s salary
(how much he/she earned for the recent past three months)
C. Certificate of the supporter’s bank balance, a copy of supporter’s bankbook
4. Supporter’s certificate of enrollment as a student
5. A copy of supporter’s residence card
6. A copy of applicant’s passport (face page)
7. One piece of photo（40 ㎜×30 ㎜）
8. Stamps (392yen)
9. An envelope (This is not necessary if you submit the documents above to International Affairs Division.)
Notes
1. As the result of examination by the immigration office, additional documents can be required depending
on the situation.
2. Additional documents can be required when the applicant applies for visa at the Japanese Embassy or
Consulate even though he/she has “certificate of eligibility”.
